Forms and custom fields
Add your own fields to records and create your operation and offense file types, with a live preview.
Every server has its own habits: a blood type on a citizen record, a file number on a warrant, a bank robbery with its own headings. In Settings › Forms, you add this information yourself, no developer needed. Every field shows up right away in the Preview, exactly as your members will see it.
What you’re configuring
Section titled “What you’re configuring”At the top of the page, pick what you want to configure. There are two families:
- Records: Citizen record, Vehicle record, Weapon record, Member record, Warrant and Medical history. Each record has a single form: the fields you add come on top of every citizen, every vehicle, every weapon… Medical history (allergies, treatments…) is only seen by the medical department: see Medical records.
- Types: Operation types, Offense file types, Medical record types and Case types. Here, each type has its own form: a bank robbery and a hostage situation don’t ask for the same information.
On a phone, you pick from the What you’re configuring list.
When you configure a family of types, the list of types sits above the editor, with each one’s number of fields. Click a type to open it. A family holds 40 types at most.

- Give it a Name and, if you like, a Description (what this type is for).
- Choose Restricted to: one specific service, or All services.
- Click New type at the bottom of the panel to create it, then add its fields.
The settings button (to the right of the type’s name) reopens this information. Delete the type works by holding the Delete button down: the type is no longer offered, but documents already written with it are kept.
Operation types
Section titled “Operation types”An operation type (bank robbery, hostage situation…) sets what an operation contains: its base sections, the roles of officers on scene and the fields of the final report. With the base setup, four types already exist for the police: Bank robbery, Jewelry store robbery, Federal bank robbery and Hostage situation.
- The base sections (suspects, hostages, location…) are renamed or turned off type by type, like any base field (see below). A section turned off disappears from operations of that type; with Hostages turned off, no statement is required any more.
- The custom fields make up the Report details, filled in on the operation’s Report tab. Their required fields only count at submission.

Section titled “Roles of officers on scene”Under the editor, the Roles of officers on scene card lists the roles an officer can hold in an operation of this type, in order (at most 12, 40 characters each). The starter roles are Operation lead, Negotiator and Fire support.
- Add a role adds one; the arrows change the order; the cross removes it.
- The Lead box marks a role that writes the final report, closes the events and submits the report. Whoever opens an operation gets the first Lead role.
- With no Lead role, only whoever opened the operation (and those with Operation reports › Edit any operation) write the final report.
Click Save. Renaming a role changes nothing for past operations: they show it under its new name.
Offense file types
Section titled “Offense file types”Offense file types show here too, with their form and a reminder of their rule (Suggested or Blocking). Their covered levels, review and legal statuses are set in the Penal code module, Case types tab: the Open the penal code link takes you there. A type created here without a level is never picked for a file.
Delegated administration › Manage forms isn’t enough for these types: creating them and editing their fields requires Offense file types › Create and edit types, deleting them Offense file types › Delete a type. Without these, the actions stay visible but locked, with a padlock and the help “Restricted: …”.
Base fields
Section titled “Base fields”Some fields exist on every record because SuperMDT needs them: a citizen’s name for search, a vehicle’s plate for a traffic stop, a report’s title… These are the Base fields. They show at the top of the editor, before your Custom fields, with a lock: they can’t be deleted or moved.

The Member record, the Warrant and Case types have no base field: everything you add there is custom.
For types (operations, offense files, medical records), base fields are set type by type: a hostage situation keeps its Hostages section while a jewelry store robbery can do without; a ticket can hide the time placed in a cell. An operation type’s Location also has its Location input: text, map or both (see the Location field).
Renaming or hiding a base field
Section titled “Renaming or hiding a base field”Click a base field to open its panel.
base-field-panel- Label: type the name your members will see (up to 60 characters), for example “Registration” instead of “Plate”. Leave it empty to keep the original label; Restore the original label clears it. In the list, a renamed field reminds you of its original name.
- Hide this field: the field disappears from forms and records (and, for the photo, from lists). Values already entered are kept: they come back if you show the field again. A hidden field carries the Hidden badge.
- A required field (the name, the plate, the title…) can’t be hidden: the switch is grayed out.
Click Save. The Preview follows your settings: renamed labels, hidden fields removed. Every change is recorded in the audit log.
Default image
Section titled “Default image”On the Citizen record, Vehicle record and Weapon record, a Default image slot follows the base fields. This image shows on every record of that type that has no photo. Drop an image or click Add a photo; Replace and Remove change or clear it. Without a default image, a neutral illustration shows (a silhouette, a vehicle, a weapon). See Photos.
Adding a field
Section titled “Adding a field”- Click Add a field.
- Write the Label: what your members see (up to 80 characters).
- Pick the Type from the grid (see the table below).
- Adjust what’s specific to the type, if anything (Type settings).
- If needed, turn on Required field, add some Help (a sentence under the field) and a Section.
- Click Add a field at the bottom of the panel.
A form holds 60 custom fields at most.

A Yes / no field can’t be required: it always has a value. Photo and Photo gallery fields take PNG, JPEG or WebP images of 5 MB at most: see Photos.
List choices
Section titled “List choices”For a List or Multiple choice field, you write the Choices one by one; Add a choice adds another. Each choice can get a color (or No color): it shows as a dot next to the label. The order of the choices is the order of the list; drag them by their handle to change it. You need at least one choice (100 at most), and two choices can’t share the same label.
Reference list fields
Section titled “Reference list fields”A Reference list field has people pick a row from one of your lists: a medical procedure, an item of a custom list… Create the list first under Reference lists, then choose it in the field.
The MDT member field
Section titled “The MDT member field”An MDT member field has people pick members of your organization, for example the officers present at an arrest or a file’s prosecutor.
- Allowed ranks: only members with these ranks are offered, across all departments (at least one rank). Tick all and Untick all save time.
- Several people: up to 10 people instead of one.
- Outside person allowed: people can also type the name of someone without an MDT account; it shows with the “outside” mention.
When reading, a name leads to the member’s profile if you can view the roster; a member who left the organization stays shown, with “left the organization”.
The Linked report field
Section titled “The Linked report field”A Linked report field has people pick an existing operation, by its number or title (the most recent ones you can read, or those matching your search). In an offense file, it does more: when saved, the file’s person joins the operation’s suspects, and the file shows in the operation. An operation the reader can’t read shows as “Report not accessible”.
The Location field
Section titled “The Location field”A Location field records where the events took place. Its Location input sets what your members fill in:

On the map, tap the spot to place the point, then drag it to adjust; Place at the center of the view and Remove the point help on a phone. Add a perimeter draws a circle around the point when the location is vague: the slider sets its radius, from 25 m to 2 km. Map tiles are your organization’s (those of the live map). When reading, the location shows with a mini map; no numeric coordinates are shown.
Sections
Section titled “Sections”Fields in the same Section are grouped under a heading, in the preview and on records. Type the section’s name: SuperMDT suggests the ones already used in the form. When other fields have a section, the ones without one end up under “Other information”.
Editing a field
Section titled “Editing a field”Click a field in the list to open its panel.
field-panel-offense- The Label, Help, Section, whether it’s required and the type settings can change at any time.
- The Type, however, is locked once the field exists: values already entered depend on it. To change the type, create a new field.
- The Technical key (for example
blood_type) is derived from the label when the field is created and never changes, even if you rename the field. It’s used by exports and integrations.
To reorder fields, drag them by their handle (or use the move up / move down buttons from the keyboard). The list order is the form order.
Deleting a field
Section titled “Deleting a field”In the field’s panel, hold the Delete button down (there is no confirmation dialog). The field disappears from records, but values already entered are kept: they come back if you recreate the field.
